All forms must be turned in to the Athletic Office BEFORE the first day of practice in order for the athlete to participate.

PIAA Physical Forms Sections 1~6
Needs to be completed once each year after June 1.
Re-Certification Forms Sections 7~8 (winter & spring sports only)
Needs completed when participating in a second and/or third sport and have already had a physical.
Sports Accident Insurance
The Punxsutawney Area School District does not purchase a Sports Accident Insurance Policy. Click here for your information only. No need to return with packet.
Dual Athlete Participation Form
This form is to be completed when opting to play 2 sports in the same season.
Coaches must approve via their signature - prior official start of practice season.
