2019 - 2020 Winter Sports Season

There will be a meeting on Thursday, October 3, at the High School for students planning on participating in a winter sport. If you can't make this meeting, you can pick up the forms in the Athletic Office at the High School or print them off the Athletic Pages.
The meeting at the High School will be held at 10:05 am in the cafeteria.

Every athlete participating in a winter sport needs the following forms filled out and returned to the Athletic Office at the High School by Friday, November 15, 2019.

  • PIAA Section 1: Personal and Emergency Information

PIAA Physical Form

  • Section 2: Certification of Parent/Guardian
  • Section 3: Understanding of Risk of Concussion and Traumatic Brain Injury
  • Section 4: Health Form
  • Section 5: PIAA Comprehensive Initial Pre-Participation Physical Evaluation and Certification of Authorized Medical Examiner


NOTE:  Students trying out for Cheerleading must have their physicals turned in before October 28, 2019.  Physicals must be dated after 6/1/19.  Tryout Practices will be held Monday, October 28; Wednesday, October 30; With Final Tryouts on Thursday, October 31All practices will be held from 3:30 - 5:30 in Gym B.


PIAA Re-Certification 

  • Section 7: Re-Certification by Parent/Guardian
  •  Section 8: Re-Certification by Licensed Physician of Medicine or Osteopathic Medicine (needed if any SUPPLEMENTAL HEALTH HISTORY questions are checked yes on Section 7)


 NOTE: Any athlete can get a physical for participation in any sport for the 2019-2020 school year at any time beginning June 1, 2019. The form used must be the P.I.A.A. physical form. NO other form can be accepted. This does not eliminate re-certification. Only one physical per year is required but that must be for the FIRST sport the athlete participates in for the new school year.
  1. You can call 814-938-1818 to schedule an appointment for a physical with Dr. Simpson.  There will be no charge for this service.
  2. You may also choose to take your child to a physician of your choice with the expense being incurred by you.
  3. All athletes participating in a winter sport must have their physicals completed and the necessary paperwork turned into the athletic office no later than Friday, November 15.
  4. Practice begins Monday, November 18, 2019.
